Nurse-Family Partnership is an evidence-based community
health program that helps transform the lives of vulnerable
mothers pregnant with their first child. Nurse-Family
Partnership is a voluntary program that provides home
visitation services to low-income, first-time mothers.
Nurses begin home visits early in the mother’s pregnancy and
continue visitation until the child’s second birthday,
providing a total of 64 visits over the two-year period.
Participants are referred to the program before they are 21
weeks pregnant. The Gift of Life Nurse-Family Partnership
currently has an enrollment of 51 young mothers
participating in the program and expects to serve 100
mothers in the coming year. |
