Nurse-Family
Partnership (NFP) is an evidence-based, nurse home visitation program
that improves the health, well-being and self-sufficiency of low-income,
first-time parents and their children.
Program Goals:

- Improve child health & development
- Improve the economic self-sufficiency of the family

Program Structure: Nurse Home Visitors (Registered Nurses) and
their clients make a 2 1/2 year commitment to one another. The number of
planned visits consist of 14 during pregnancy, 28 during infancy, and 22
during toddler-hood.
The Nurse Home Visitors' caseloads do not exceed 25 families per
nurse. The clients are referred to this program by the Gift of Life
Maternity Care Program Care Coordinators.
